iHealth
iHealth is a statewide collaborative of community-based organizations united to advocate and negotiate on behalf of HIV Targeted Case Management Programs and the chronically ill Medicaid recipients we serve with the Health Home system, and represent those programs within governing bodies of Health Homes. Collectively, iHealth represents 27 providers throughout New York State.
APICHA Community Health Center
Our Mission is to improve the health of our community and to increase access to comprehensive primary care, preventive health services, mental health and supportive services. We are committed to excellence and to providing culturally competent services that enhance the quality of life. We advocate for and provide a welcoming environment for underserved and vulnerable people, especially Asians and Pacific Islanders, the LGBT Community and individuals living with and affected by HIV/AIDS.
Argus Community
ACCESS is one of longest NYSDOH approved Care/Case Management providers serving the Bronx and upper Manhattan, recognized as CM experts by community partners, clients, and the DOH/AI. We are available 24 hours to connect members & loved ones with the best medical-well care or drug treatment services while helping with housing/ legal/court problems, HASA, PA, SSI benefits, recreation and any barriers preventing independence.
Ascnyc
Allied Service Center NYC (ASCNYC) annually helps more than 14,000 men and women living with chronic illnesses – including drug or alcohol abuse, Hep. C, Diabetes, Asthma, High Blood Pressure, Depression, Anxiety and HIV/AIDS — make lasting positive changes towards health, housing and self-sufficiency. Our innovative peer education, access to healthcare, and safe-practice counseling services promote empowerment, stability, and connection to community.
Bailey House
Bailey House provides Housing, Care Coordination and Mental Health services for people living with HIV/AIDS, and other chronic illness including those who are at Risk or Status unknown. Bailey House’s programming strives to reduce HIV incidence, increase access to healthcare, optimize health outcomes, and reduce health disparities and homelessness.
BOOM! Health
BOOM!Health is a merger of equals between Bronx AIDS Services and CitiWide Harm Reduction creating an organization with a seamless continuum of services providing care coordination, harm reduction counseling and other support services for vulnerable youth and adults in NYC presenting with complex conditions such as mental illness, HIV/AIDS, Hepatitis C, hunger, addiction, diabetes, hypertension, and many other chronic conditions.
Bowery Residents Community
BRC recognizes that the effort to end homelessness requires more than passion and experience, but also a sense of organizational responsibility and the strength to manage professionally. In the 45 years that BRC has provided housing and treatment services to homeless adults in New York City, we have demonstrated continuing expertise in developing and delivering services with efficiency and positive outcomes.
Brightpoint Health
Brightpoint Health operates one of the largest and most experienced Medicaid Case Management Programs in the State of New York and serves patients throughout New York City. Integrated primary care, behavioral health services and dental care are also offered through its various Diagnostic and Treatment Centers to promote continuity of care and ease of access to care. Medicaid and most insurances are accepted.
Community Health Action of Staten Island (CHASI)
The mission of Community Health Action of Staten Island is to improve the lives of individuals, families and communities challenged by health disparities related to poverty, discrimination and lack of access through direct service, education and advocacy.
GMHC
GMHC is a not-for-profit, volunteer-supported and community-based organization committed to leading the fight against AIDS on a national platform. We provide prevention and care services to men, women and families that are living with, or affected by HIV/AIDS in New York City. We also advocate scientific, evidence-based public health solutions for hundreds of thousands worldwide. Our mission: GMHC fights to end the AIDS epidemic and uplifts the lives of all affected.
Harlem United
Harlem United is a community-based organization providing a unique continuum of care. We integrate socially and economically disenfranchised people into a healthy and healing community. We also offer our clients access to a full range of medical, social, and supportive services. The majority of our clients are people living with HIV/AIDS whose diagnoses are often complicated by addiction, mental illness, and homelessness. Many have also faced significant barriers to care due to poverty, race, HIV status, sexual orientation or gender identity.
Housing Works
Housing Works is a healing community of people living with and affected by HIV/AIDS. The organization’s mission is to end the dual crises of homelessness and AIDS through relentless advocacy, the provision of lifesaving services, and entrepreneurial businesses that sustain our efforts. Housing Works has a network of three primary health medical centers and health Home programs in the Bronx, Manhattan, and Brooklyn. Our Health Home Care Management program can compliment any other services at Housing Works, or it can be used as a stand-alone service. Health Home services are most effective when used together as part of an integrated Healthcare model.
