1. Communication and Coordination

  • Timely Acceptance of Patient Referrals from Care Managers
  • Information Exchange with Care Manager
  • Participation in Case Conferences
  • Informing and Utilizing Care Plans

2. Use of Health Home Health Information Technology Platform

  • Utilize IT platform to access patient data and patient Care Plan
  • Utilize IT platform to alert care team of incidents or relevant patient updates
