A Health Home is a care management model whereby all of an individual’s caregivers communicate with one another so that all of a patient’s needs are addressed in a comprehensive manner. This model is achieved through the use of “Care Managers” who oversee and coordinate the comprehensive health care needs of an individual. Community Care Management Partners (CCMP) Health Home is a community-based health home comprised of eight partners (Argus Community, Community Healthcare Network, iHealth, Institute for Family Health, Mount Sinai Hospital, Brightpoint Health, Visiting Nurse Service of New York and Urban Health Plan). The CCMP Health Home provider network includes nearly 100 highly experienced organizations such as hospitals, primary and specialty care medical providers, outpatient mental health care and substance use providers, housing providers and community-based social services organizations focused on providing quality services in the most convenient and comfortable manner, to its members.
