Data & Analytics

Monthly quality and performance metrics, daily reports to manage workload within our Electronic Health Record, annual outcomes analysis to assist with advocacy efforts and marketing, machine learning fed predictive models for early detection of emergent needs.

Training

Up to 30 hours of live trainings for new staff, a Learning Management System to track training progress, and multiple training methods available. CCMP is the only Health Home with trainings that fully integrate state policy requirements with Electronic Health Record functionality, presented in a way that Care Managers can understand.

Program Support

Dedicated Quality Improvement Specialist to support your agency through Onboarding into the CCMP Network, review initial enrollments, and provide ongoing immediate support when needed.
